Adding a New Patient

📝Add New Patient
BotMD Care allows easy patient enrolment via various form-types such as (FormSG, Typeform etc). New Patient can be added from 2 places in the Dashboard.
1. First place it can be added from, Click on Add New Patient 📝 from the left hand-side panel in the dashboard.
2. Second place "New Patient" can be added from is in "Patient List" Module. Click on Add patient 📝
3. Clinical users will be directed to the secure enrollment form to register the patient (Example illustrated below - Form SG). Fill in Home Monitoring Patient Registration Form.
Note: Enrollment forms are customized according to each institution's needs and may therefore vary depending on the type of form used (we support Google Forms, Typeform, FormSG to name a few).
3. Select your preferred Monitoring Form (Programme).
4. Choose patient's preferred Language and fill in the patient's details.
Note: This can be defaulted to a preferred language and therefore would not need to appear in the enrollment form.
5. Select a Reminder Method for the patient. This can also be defaulted to a preferred messaging platform and therefore would not need to appear in the enrollment form.
6. Choose the Clinical User In-Charge. The Clinical User In-Charge is the person who is directly responsible for the care of the patient. Note that the Clinical User in Charge does not receive any clinical alerts.
7. Choose the Clinical User to Alert. This is the clinical user who will receive clinical alerts via messaging platform (WhatsApp, Viber, SMS, etc). You may choose the same person selected as Clinical User In-Charge if desired.
8. Select a schedule for your patient to receive submission reminders. This can also be defaulted to a pre-set reminder frequency and time and therefore would not need to appear in the enrollment form.
9. Finally, submit the Form to add the patient to the "Active Patients" tab in the "Patient List" module.
7. Patients will receive an enrolment message containing the monitoring form link via SMS/Whatsapp/Viber
