Adding a New Patient

📝Add New Patient
BotMD Care allows easy patient enrollment via various form-types such as (FormSG, Typeform etc).
1. To add a new patient, tap on the Hamburger then Add New Patient 📝
2. Clinical users will be directed to the secure enrollment form to register the patient (Example illustrated below - NKTI Hemodialysis).
Note: Enrollment forms are customized according to each institution's needs and may therefore vary from what is shown here.
3. Select your preferred Monitoring Programme / Form
4. Choose patient's preferred language.
Note: This can be defaulted to a preferred language and therefore would not need to appear in the enrollment form.
5. Fill in the patient's details
6. Choose the Clinical User in Charge (person in charge of looking after the patient) and the Clinical User to Alert (to be alerted when submitted readings are not within the threshold).
Note: that the Clinical User to Alert is different from the Clinical User in Charge in that the latter does not receive any clinical alerts.
7. Select a Reminder Method for the patient. This can also be defaulted to a preferred messaging platform and therefore would not need to appear in the enrollment form.
8. Make sure you have obtained consent from your patient. Tap Submit to save and submit the form.
9. Patients will receive an enrollment message containing the monitoring form link via SMS/Whatsapp/Viber
