Discharging and Re-Enrolling Patients

If you have a patient who will no longer require monitoring via the Bot MD Care Platform you can discharge your patient from the platform. Patients who are discharged will no longer receive reminders to submit their monitoring forms.

Discharging a Patient

1. In the Overview page, tap on Active Patients. You will be directed to the Active Patients tab in the Patient List.

2. Scroll down to look for your patient in the list. You may enter your patient name in the search field for a quicker search. When you have located your patient, tap on More settings then select Discharge Patient.

3. A confirmation message will appear prompting you to confirm your patient's discharge. Tap on Discharge to complete your Patient Discharge from the Monitoring Platform. Your patient will receive a message informing her/him that she/he has been discharged from the monitoring program.

Note : Patients who are discharged will no longer receive reminders to submit their monitoring forms. However, the patient's submission data (including graphs and tables) will still be available for viewing and downloading in the Patient List under Discharged Patients.

Re-Enrolling a Patient

There may be instances where a discharged patient may require monitoring again with the Bot MD Care platform. These patients can be re-enrolled in a few simple steps.

1. Tap the Hamburger icon then  Patient List.

2. Tap on the Discharged column to access a list of Discharged Patients.

3. Scroll down to look for your patient in the list. You may enter your patient name in the search field for a quicker search. When you have located your patient, tap on More settings then select Enroll Patient.

4. A confirmation message will appear prompting you to confirm your patient's enrollment. Tap on Enroll Patient to complete your Patient Re-Enrollment into the Monitoring Platform. Your patient will receive a message welcoming her/him again into the monitoring program and will once again receive reminders to submit monitoring forms as previously configured.
