Discharging and Re-Enrolling Patients

If you have a patient who will no longer require monitoring via the Bot MD Care Platform you can do two things:

1. Remove (discharge) your patient from a monitoring program.

  • Use this option if you want to remove a patient from one or more monitoring programs 
  • Patient is kept active in the dashboard so you can still send them messages.

2. Deactivate your patient.

  • Use this option if you want to remove a patient from all monitoring programs. If the patient is enrolled in a program, he/she will receive a message informing them that they have been discharged.
  • The patient will no longer receive reminders, and you can no longer send them messages
  • The patient's graphs and submissions will still be available for download under Patient List

Removing a Patient from a Program (Discharging a Patient)

1. In the Overview page, click on Active Patients. You will be directed to the Active Patients tab in the Patient List (alternatively, you may click on Patient List on the left nav panel).

2. Search for your patient then click on the More Settings icon (small three dots) then select "Remove program."

3. Select the program from which you wish to remove your patient then click on Remove program.


Note : Patients who are removed from a program will no longer receive reminders to submit their monitoring forms; however, a patient will continue to receive reminders from any program he/she is still currently enrolled into. The patient's submission data (including graphs and tables) will still be available for viewing and downloading in the Patient List under Active Patients.

Adding a Patient to a Monitoring Program

There may be instances where a patient needs to be enrolled in an additional monitoring program apart from his/her current one, or when a discharged patient may require monitoring again with the Bot MD Care platform. These patients can be re-enrolled in a few simple steps.


1. In the Overview page, click on Active Patients. You will be directed to the Active Patients tab in the Patient List.

Alternatively, you may click on Patient List on the left nav panel


2. Search for your patient then click on the More Settings icon (small three dots) then select Add program.

3. Select the program in which you wish to enroll your patient. 

4. Click on Enroll Patient

5. A message will appear confirming your patient's enrollment into the program.

Re-Activating a Patient

There may be instances where a deactivated patient may require monitoring again with the Bot MD Care platform. These patients can be re-activated in a few simple steps.

1. In the Clinical Dashboard, click on Patient List then click on the Deactivated column to access a list of Deactivated Patients.

  1. Search for your patient then click on the More Settings icon (small three dots) then select "Re-enroll patient."

  1. Click "Activate."

4. Your patient will receive a message welcoming her/him again into the monitoring program and will once again receive reminders to submit monitoring forms as previously configured. In addition, your patient will appear again in the Active Patients tab.
